This Plan is FOR THE WHOLE FAMILY. It includes an Accidental Death & Dismemberment Benefit: with up to $10,000. Accident Medical / Dental Expense Benefit: Up to $5,000 which pays for medical expenses resulting from a covered accident up to the maximum benefit amount per covered incident.
